What is Medical coding?
In layman terms, it is a process of capturing the story that unfolds in a clinical setting to CODES.
Let’s have this scenario: Imagine you have a knee pain after playing football and went to a Dr in your vicinity.
The Dr examines the painful right knee and administers a pain killer in the form of an injection and prescribes some medications as part of general management.
A complete medical record is created and YOU the future Medical Coder will capture this story as M25.561, 96372, J3301.
These are standard transaction code-sets derived from 3 different manuals based on well-defined rules and conventions.
Diagnosis – Right knee pain – M25.561 – ICD 10 CM
Procedure – Injection – 96372 – CPT 4
Medicine – Pain killer – J3301 – HCPCS
What is Medical Billing?
The codes are subsequently processed by Medical Billers for generating payment for Clinical and medical services to the patient’s Insurance company.
The claim adjudication and determination is done by Medical Claims Examiners based on Eligibility Verification, provider credentials and Medical Necessity.
There are 2 outcomes – either the claim is paid or denied. If denied the reason for denial is known and it can be appealed with the relevant information.
We can expect the payment of $37 for each item as follows. This reimbursement is subject to payer variations, state-wise differences,
contractual obligations etc.

I have been with the US Healthcare industry for over a decade and educated and enlightened Surgeon, Physicians, Nurses and medical staff and medical coders and medical billers.
I did my Bachelor of Psychology from Madras University and Master of Psychotherapy and counseling from the Institute of Psychotherapy and Management Sciences and also completed the Diploma of Bach Remedies.
Later on I pursued my medicine degree (Bachelor of Homeopathic Medicine and Surgery) from Venkateswara Homoeopathic Medical College in Chennai and did my M.D. Acupuncture.
Having entered the medical coding industry, Dr Ramasubbu tried completing the 33 Certification exams from the American Academy of Professional Coders, based in USA. I is the All-India topper of AAPC and served as the Speaker of AAPC, Chennai Chapter in 2016 and 2018.
I provided the seminars on Active Wound Care Management and CPT code changes of 2018.
I is the Certified Compliance Officer and Medical Auditor and acting as a Consultant medical billing company based in San Francisco, California and the Managing Director of DR. RAMS MEDICAL CODING AND BILLING ACADEMY through which thousands of students have completed the Advanced Medical Coding Training, CPC and CPMA (Medical Auditing) exam training and have transformed to successful certified Medical Coders and Medical Auditors.
